Shalom Village Privacy Policy

Our Commitment to you at Shalom Village

At Shalom Village we ensure that personal information from our residents, staff and volunteers complies with PIPEDA. Shalom Village provides residents and staff with knowledge of what information is being collected, why it is being collected, and how the information will be used, disclosed and protected while someone is a resident or a member of the staff/volunteer team. We aim to guarantee that while you are A.T. H.O.M.E. at Shalom Village, you can rest assured that your information is secure at home with you!

Personal information includes:

  1. Personal address, telephone number, email address, or contact information

  2. Any identifying number assigned to an individual which can lead to their identification (e.g. Social Insurance Number)

  3. Financial information including income, assets, bank statements, rent payments, and rent payment history

  4. Information about race, national, or ethnic origin, citizenship status, colour, religion, age, sex, sexual orientation, marital or family status of an individual

  5. The education, medical, psychiatric, psychological, criminal, or employment history of an individual

  6. Employee information including resumes, salary and benefits, disciplinary action, bank information, or recorded problems at work

Personal information does NOT include:

  1. The name, position, and business phone number of employees

  2. Statistical data that does not identify any specific individuals

  3. Business contact information and certain publicity available information (such as published in a public phone directory)

The privacy principles used at Shalom Village

Shalom Village complies with the Federal Privacy Act that includes the 10 privacy principles under the Personal Information Protection and Electronic Documents Act (PIPEDA)

1. Accountability

Employees at Shalom Village are responsible for the personal information in their position. Shalom Village ensures that policies are in place, and that staff receive education to comply with the PIPEDA principles.

2. Identifying Purposes

The purpose behind the collection, use and disclosure of personal information will be identified by the staff member or in the documentation that is filled out by the resident or new employee.

3. Consent

Residents and staff have knowledge and consent for the collection, use and disclosure of personal information. Informed consent can be expressed in written or oral manner depending on the information requested.

4. Limiting Collection

Personal information collected by Shalom Village is strictly limited to the purposes identified and specified by the home. In the future, if Shalom Village wishes to use the information for another means, individual consent will be obtained again. All information will be collected in a fair and lawful way.

5. Limiting use, disclosure, and retention

Personal information will only be used for purposes disclosed by Shalom Village unless consent is given by the individual or requested by law.

6. Accuracy

Information collected for purposes at Shalom Village will be kept up-to-date and accurate, and changed accordingly when necessary.

7. Safeguards

The information Shalom Village collects physically, electronically, and orally for residents and staff is kept protected and secure under appropriate security measures, including education programs, locked cabinets, security passwords, internet firewalls, encryption, and in adherence to confidentiality agreements.

8. Openness

Any policies and procedures that Shalom Village uses to manage personal files/information are available to staff and residents alike.

9. Individual Access

Upon request, a staff member or resident at Shalom Village can see his or her personal information on file. A staff member or resident can challenge the accuracy and completeness of their information and have it amended as appropriate.

10.Challenging compliance

An individual can address a challenge concerning compliance with the principles above. Shalom Village has procedures in place to receive and respond to such complaints, and will take appropriate measures when necessary.

Private Policy at Shalom Village

It is our policy to ensure that under the Federal Privacy Act PIPEDA (personal information protection and electronic act) we comply to protect and secure all personal information in a thorough manner. As part of this act, residents are provided with knowledge of their privacy rights and the collection, use and disclosure of their personal information.

Our procedure:

  1. Collect information from residents/staff that is limited to purposes identified by ShalomVillage.

  2. Obtain consent during the collection of information from residents/staff.

  3. Staff will only use and disclose information about residents for the purpose for which it was collected.

  4. Personal files will be kept updated, completed, and accurate.

  5. Safeguards will be installed to protect the information in the possession of authorized staff at Shalom Village. This includes signing a confidentiality agreement, education programs, computer passwords, encryption, and secured file cabinets.

  6. The residents and staff are to be educated on the management of their personal files. This includes information about who has their files, where they are kept, the use of their files, and any information related to Shalom Village=s privacy standards and policies.

  7. All forms requiring privacy information filled out by staff, residents or volunteers of Shalom Village will include a statement of our commitment to privacy.

"The information disclosed in this document will be protected under the Federal Privacy Act PIPEDA - Personal Information Protection and Electronic Documents Act."
