The LHIN 4 Nurse Led Outreach Team is a
dynamic geriatric team including nurse practitioners, geriatricians and other
professionals skilled in the care of frail seniors. Specialized health care is
provided to residents of Hamilton area Long Term Care Homes, which not only
improves healthcare access to residents but also reduces the need for hospital
visits. This innovative
project is also utilized by students and researchers from a variety of
University and College programs, including the Family Medicine Care of the
Elderly Program at McMaster University.
