The LHIN 4 Nurse Led Outreach Team is a
dynamic geriatric team including nurse practitioners, geriatricians and other
professionals skilled in the care of frail seniors. The team provides specialized health care to residents of Hamilton area Long Term Care Homes, which not only
improves healthcare access to residents but also reduces the need for hospital
visits.
This innovative
project is also utilized by students and researchers from a variety of
University and College programs, including the Family Medicine Care of the
Elderly Program at McMaster University.

The HNHB Nurse-Led Outreach Team provides support to long term care home (LTCH) residents in their homes while promoting appropriate utilization of Emergency Departments and decreasing length of stay for hospital inpatients from LTC as primary foci.
The incumbents will work as part of the broader LHIN-wide Outreach Team and successful NP candidates will be providing chronic and episodic care to LTCH residents, provide consultation for LTCH staff and residents' families, implementing clinical pathways, and participate in annual education for staff in LTCH as part of a LHIN wide structure. As part of this team, the RN incumbents will provide consultation at multiple Long Term Care Home and community partner sites over the week. Laurie Angle, BScN RN(EC)
Laurie is a Nurse Practitioner (NP) with the Nurse Led Outreach Team in Niagara. Presently, she covers the Long-Term Care Homes located within Port Colborne, Welland, and West Lincoln.
Laurie has been an NP since 2004. Her first NP position was with the Hamilton Long-Term Care Project located out of Shalom Village. Since June 2007, she has been employed by the Niagara Health System as part of their outreach service to long term care communities across Niagara.

Jennifer Burgess, RN BScN GNC(C)
Jennifer is a McMaster University School of Nursing Alumni, and achieved her Canadian Gerontological Nursing Certification in 2007. Jennifer comes to the team with all of her passion and experience in Long-Term Care Nursing, most recently at Shalom Village. Jennifer has been involved with the Gerontological Nursing Association, Hamilton Chapter, as President in 2009. Jennifer provides education, consultation, collaboration and capacity building opportunities to LTC Homes and community partners in the HNHB LHIN.

Christine Fric, BScN RN(EC)
Christine comes to NLOT with many years of varied nursing experience. She came from the Niagara Region, where she began her nursing career in the diploma program at the McMaster School of Nursing. She proceeded to continue part time studies while pursuing her nursing career as a general staff nurse in a hospital setting. She then graduated with her BScN from Niagara University, Lewiston, NY. Christine spent many years in community nursing with the VON as well as a Public Health Nurse. She functioned in several roles such as; educator, facilitator, and consultant .Working with patients across the age continuum with a variety of health issues, health promotion and illness prevention was the main focus based on the determinants of health.
After graduation from the Primary Care Nurse Practitioner Program in 2002, Christine spent several years working in the community in primary care. Her interest in geriatrics peaked several years ago when she endeavoured to work in long term care homes, treating residents to avoid unnecessary transfers to the hospital. Christine has been working in long-term care since.

Laura-Dawn Moule B.A., M.A., M.S.N., RN(EC)
Laura-Dawn is originally from Montreal. She got her Bachelor of Arts degree from McGill University, her Master of Arts degree from the University of Sheffield and her Master of Science degree in Nursing from Vanderbilt University where she was inducted into the Sigma Theta Tau International Honor Society of Nursing. She has mostly worked in primary health care, at first in rural areas of Tennessee and then in larger cities including Burlington and Hamilton.
She has also worked in acute/urgent care settings with her most recent experience including the Emergency Department at the Greater Niagara General Hospital. She worked as a sessional lecturer at McMaster University Medical Centre’s School of Nursing as well as a preceptor for nurse practitioner students. She currently works with the geriatric outreach program through NHS.

Heather Prescott, BScN, MSc, RN(EC)
Primary Health Care Nurse Practitioner
Heather is a graduate of McMaster’s first combined MSc-PHC NP program (2009) and is a “grow-your-own” NP with John Noble Home, Brantford and the Nurse Led Outreach Team in Brant.
She comes to LTC with 5 years experience in emergency nursing. Her LTC clinical expertise lies in diabetes and pain management.
Heather provides comprehensive primary care, clinical consultation, education and support, chairs committees and participates in best practice policy development and program implementation. |

Nora Van Dalen BScN RN(EC)
Nora is a Nurse Practitioner with the Hamilton branch of the Nurse Led Outreach Team. She covers all of Burlington region and a few homes in downtown Hamilton. Nora has extensive experience both cardiac critical care and emergency nursing. Nora is passionate about the care of the elderly. In 2007 Nora received a Registered Nurses Association of Ontario (RNAO) advanced practice clinical fellowship to design an interdisciplinary approach to screen for delirium, depression, dementia in the elderly in-patient hospital population.
Nora started her nursing career in China as a field project coordinator for the University of Ottawa, School of Nursing. In this role, Nora was involved in the planning of a home based palliative care program. Upon her return to Toronto, she worked in nursing roles at Baycrest and St. Michael’s Hospital.
