Convalescent Care at Shalom Village

Shalom2Home, Goldie's2Home programs help people move back home

Shalom2Home

The Shalom2Home Convalescent Care program is a partnership between Hamilton Health Sciences, St. Josephs Healthcare, Shalom Village, Townsview Lifecare Centre, Hamilton CCAC and St. Peter's Hospital. The goal of the program is to help prepare people to return home after a stay in hospital. When the acute phase of illness has passed the care team may recommend the Convalescent Care Program as the next step in a person’s return home. The Shalom2Home Program focuses on helping participants rebuild strength, reduce their need for assistance and reinforce their independence.

The Shalom2Home team works with participants to develop a program to help them build strength and manage their activities of daily living. Participants wear their regular clothes and help prepare their own meals in the dining room. They participate in a variety of recreational and rehabilitative activities. They are active participants in their own recovery.

The Shalom2Home program is here to provide the support and environment needed to help people recovering from illness pr injury achieve their goal of returning home to an independent living arrangement. The team will work in partnership with participants, their families, physicians and the Community Care Access Centre to help them regain strength, improve functioning and build confidence to ensure a smooth transition as they move back to their homes from our home.

Admission is coordinated by  the Hamilton Community Care Access Centre. Please call (905)523-8600 ext. 2203 for more information.

Goldie's2Home

Goldies2Home is a unique day program helping people return to independent living at home after early discharge from acute care, rehabilitation or convalescent care programs. Many people are better served by continuing their recovery in their own homes with support. This ambulatory convalescent program, partnered with enhanced CCAC services, will allow you to continue your convalescence from the comfort of your own home. Your program will include nursing, physiotherapy, recreation and other support to help you rebuild strength and regain skills to reduce your need for assistance. It is a community based rehabilitation program paced to your needs and promoting independent living.

SUPPORT

The Goldies2Home team includes a CCAC case manager, registered nurse, personal support worker, physiotherapist, physiotherapy assistant, occupational therapist, speech & language pathologist and therapeutic recreationists. A ratio of 1 staff to 5 participants ensures that you get the support you need. The program is enhanced by in-home support, and is geared to the needs of people with limited endurance. Information sessions and support are offered to participants and family caregivers to ensure your move home is successful.

ACTIVE PROGRAM

The Goldies2Home program operates from 9:00 a.m. to 5:00 p.m. from Monday to Friday. You may attend from 1 to 5 days per week, depending on your needs, for a maximum 4 week period.

They have access to all amenities offered at Shalom Village including The Club Fitness Centre, Bubbi Bessie’s Café, Vicki’s Hair Salon and the Classic Theatre.
