Nurse Practitioners (2.0 FTE) and Registered Nurses (3.0 FTE)
The HNHB Nurse-Led Outreach Team provides support to long term care home (LTCH) residents in their homes while promoting appropriate utilization of Emergency Departments and decreasing length of stay for hospital inpatients from LTC as primary foci.
The incumbents will work as part of the broader LHIN-wide Outreach Team and successful NP candidates will be providing chronic and episodic care to LTCH residents, provide consultation for LTCH staff and residents' families, implementing clinical pathways, and participate in annual education for staff in LTCH as part of a LHIN wide structure. As part of this team, the RN incumbents will provide consultation at multiple Long Term Care Home and community partner sites over the week. Hospitality Coach – Food Service Manager
This role, Hospitality Coach is an excellent opportunity for a creative and enthusiastic leader. The ideal Coach is a graduate from a recognized program in Food Service Management or equivalent, has experience in LTC and is eligible for membership in the Canadian Society of Nutrition Management. Knowledge of a Kosher kitchen is helpful but not required.

- Red Seal Certificate
- Minimum two year cooking experience in Long Term Care or related field
- Hospitality Qualification, Chef ,or Cook papers
- Food Handling Certificate
- Good verbal and written communication skills
Duties:
- Work in a Kosher environment, observing Jewish dietary laws
- Observe safety and hospitality policies and procedures
- Take leadership in the kitchen, giving effective direction to other team members
- Prepare and serve daily meals independently, according to standard requirements
- Handle multiple tasks
- Co-operate and work closely with Supervisor, Dietary Aides, Cooks and other staff
