ambulatory care program
Goldie's Place
is an active Day Program for adults living in the community who
have physical or cognitive impairments. From Monday to Thursday, various
recreational activities are provided, including lunch and snack, and assistance
where necessary. Goldie's Place provides a secure, friendly environment,
promotes health maintenance, and provides opportunities for socialization and
caregiver relief.
Transportation is available for people
living within Hamilton, including
those with wheelchairs.
Goldie's Place
provides a home away from home for adults who benefit from regular interaction
with health and rehabilitative professionals. Activities are flexible and varied
to meet the diverse needs of our participants. Social and health promotion
programs are provided in concert with the Ministry of Health and Long Term Care
Adult Day Program standards.
Goldie's Place provides peace of mind
for families - allowing them time for work and personal activities.
Caregivers at home are afforded respite from responsibilities that can be
overwhelming at times.
Goldie's Place
is an integral component of the Shalom Village community and participants have
access to the many services available, including The Club Fitness Centre,
Bubbi Bessie's Cafe and The Classic Theatre. By participating in
stimulating activities, Goldie's participants' quality of life is enriched and
their scope of experiences is expanded. We

Goldie's2Home
Goldies2Home is a unique day program helping people return to
independent living at home after early discharge from acute care,
rehabilitation or convalescent care programs. Many people are better served
by continuing their recovery in their own homes with support. This
ambulatory convalescent program, partnered with enhanced CCAC services, will
allow you to continue your convalescence from the comfort of your own home.
Your program will include nursing, physiotherapy, recreation and other
support to help you rebuild strength and regain skills to reduce your need
for assistance. It is a community based rehabilitation program paced to your
needs and promoting independent living.
SUPPORT
The
Goldies2Home team includes a CCAC case manager, registered nurse,
personal support worker, physiotherapist, physiotherapy assistant,
occupational therapist, speech & language pathologist and therapeutic
recreationists. A ratio of 1 staff to 5 participants ensures that you get
the support you need. The program is enhanced by in-home support, and is
geared to the needs of people with limited endurance. Information sessions
and support are offered to participants and family caregivers to ensure your
move home is successful.
ACTIVE
PROGRAM
The
Goldies2Home program operates from 9:00 a.m. to 5:00 p.m. from Monday to
Friday. You may attend from 1 to 5 days per week, depending on your needs,
for a maximum 4 week period.
Participants
enjoy an active program with many opportunities for social interaction and
recreational programs in addition to rehabilitation services. They have
access to all amenities offered at Shalom Village including The Club
Fitness Centre, Bubbi Bessie’s Café, Vicki’s Hair Salon and the
Classic Theatre. Baths or showers are available, with assistance as
needed, and professional foot care can be provided. A hot lunch and healthy
snacks are served, with participants actively involved in meal preparation
to help them prepare for independent living at home.
TRANSPORTATION
Wheelchair
accessible transportation to and from Goldies2Home is available.
COST
The
Goldies2Home program is funded by the Ontario Ministry of Health and
Long Term Care. There is no cost to participants.

Goldie's2Home is a collaborative partnership
including Shalom Village, Hamilton Health Sciences, St. Peter’s Hospital,
the Hamilton Convalescent Care Program and the HNHB Community Care Access
Centre (CCAC). Referral to the
Goldie's2Home
day program is through the Hamilton Niagara Haldimand
Brant Community Care Access Centre (CCAC). Your treatment team will assess
your needs and discuss your recovery and discharge plan with you during the
early stages of your acute or rehabilitation treatment in hospital.
